Immunization Exemptions in Tennessee?

We are considering moving to TN and want to know if there are exemptions to having our children immunized. Our state has 3 exemptions and the only thing you have to do is on the form that you fill out for school, on the back it has a place where you sign your exemption. No questions asked. Is there anyone out there who lives in TN that can tell me what the law is or what your experience is?
